1. Fragmented Data Infrastructure

What to do: Before scaling any AI initiative, invest in a unified data layer. This does not mean a multi-year data warehouse migration. Start with a data mesh or federated approach that establishes consistent standards and APIs across the systems your AI applications need to access.
2. No Cross-Functional Ownership
Pilots are typically owned by a single team, often IT or data science. Scaling requires buy-in and active participation from operations, finance, HR, legal, and the business units that will actually use the system daily.
What to do: Establish an AI Center of Excellence (CoE) with representatives from each major business function. The CoE should own the scaling roadmap, prioritize use cases based on business impact, and serve as the bridge between technical teams and end users.
3. Change Management Is an Afterthought
The most technically impressive AI system will fail if people do not use it. IBM's experience is instructive here: when they rolled out agentic AI across their 270,000-employee organization, they paired the technology with extensive training and workflow redesign. The result was an estimated $4.5 billion in productivity gains, with their AskHR system resolving 94% of routine employee questions and managers completing tasks like promotions 75% faster.
Most companies skip this step. They deploy the tool and expect adoption to follow.
What to do: Budget at least 20% of your AI project cost for change management. This includes training, workflow documentation, feedback loops, and dedicated internal champions in each department.
4. Undefined Success Metrics
Pilots often measure success with vague criteria like "it works" or "users like it." Scaling requires hard metrics tied to business outcomes: cost reduction, revenue impact, throughput improvement, error rate reduction, or time saved.
Deloitte's research shows that organizations with clear metrics see dramatically better results. Among companies that track AI ROI rigorously, 88% report measurable revenue increases and 87% report cost reductions.
What to do: Define three to five quantitative KPIs before you start scaling. Establish a baseline measurement, set targets for 90 and 180 days post-deployment, and build automated dashboards to track progress.
5. Regulatory Uncertainty Creates Paralysis
The regulatory landscape for AI is shifting fast. Colorado's SB 205, effective since February 2026, requires impact assessments for high-risk AI systems. California's SB 53 mandates transparency reports from frontier model developers. The EU AI Act's high-risk system requirements take full effect in August 2026.
Many companies respond to this uncertainty by slowing down or stopping AI initiatives entirely. This is the wrong approach.
What to do: Build governance into your scaling framework from day one. Conduct impact assessments for every AI system you deploy, document your data sources and model decisions, and establish a review process for high-risk applications. Companies that build compliance into their AI pipeline now will have a significant advantage over those scrambling to retrofit governance later.
A Practical Framework for Closing the Gap
Based on what we see working across industries, here is a five-step framework for moving from pilot to production:

Audit your pilot portfolio. List every AI pilot and proof of concept in your organization. For each one, document the business case, current results, data dependencies, and scaling requirements. Kill the ones that do not have a clear path to ROI.

Prioritize ruthlessly. Rank your remaining initiatives by two criteria: business impact and scaling complexity. Start with high-impact, low-complexity use cases. PepsiCo's approach is a good model here. They used AI agents with digital twins to identify 90% of potential issues before physical modifications, delivering a 20% increase in throughput on initial deployments.

Fix the data layer first. Do not scale an AI application on top of broken data infrastructure. Invest the time to establish consistent data standards, quality checks, and access patterns.
